THE INTERDISCIPLINARY TEAM.(Nursing)

What is Interdisciplinary Team?.

This is a group of healthcare professionals (that you as a Registered nurse is part of ) with different areas of expertise in their field.Each background compliment the other.They work together with a common interest of improving the health of the patient( s). It is a complex process that involve more than two healthcare professionals.They take care of the clients(patients) through assessment, planning, implementation and evaluation. And with a good outcome for the patients in mind.

Who are these People ?.

They consist of:

The physicians (doctors)

Registered nurses.

Pharmacists

Respiratory therapists.

Physical therapists.

Occupational therapists.

Speech Therapists.

Case Managers and Social workers.

Registered Dietitians.

They communicate with each others to promote continuity of care for the patients and families.

The Patient Education.

Throughout hospitalization consistent patient education using demonstration and return demonstration is very important. One to one teaching is very important.Patient education should be individualized.

BROCHURES and other printed materials given to patients. Many hospitals uses Krames and other recommended educational tools.

Members of Medical team.
