PRESSURE ULCER.

Who develops pressure ulcer.

Not me I am walking around.

If you are alert, oriented and Ambulates.

Can you develop pressure ulcer.

The answer is No.

If you are able to comprehend, not incontinent change position.

Will you develop pressure ulcer.

The answer is No.

So if you are compromised.

Stay in bed for too long.

Comfort and redistribution.

Or in one position for too long.

And you cannot reposition yourself.

Yes pressure ulcer will visit you.

You are bed ridden.

Reposition for Comfort.

Reposition for pressure redistribution.

You are wheelchair bound.

There is limitation in the flow of blood.

In the area where there is pressure.

How do you predict pressure ulcer risk.

Bedsores will develop.

Pressure ulcer develops in bony prominent areas.

Pressure ulcers develop in stages.

Stage 1( Nonblanchable)

Stage 11 ( Partial thickness)

Stage 111 ( Full thickness ,skin loss,damage to the subcutaneous tissue without affecting the bones )

Stage 1V ( Full thickness pressure ulcer deepens with damage to the subcutaneous tissue, muscle, bones and could sometimes get to the tendons and joints)

Physical Therapist involvement.

WOUNDCARE team involvement.

Nutrition consultation.

Stage 111 must be reported.

If developed while in the hospital.

Report to your manager.

Manager will report to the State.

Position change and monitor.

Please don’t neglect your responsiblity.

Reposition for comfort.

Reposition for pressure redistribution.

Understand the stages of pressure ulcer. Expensive to treat, plus the suffering.

Body image disturbance.

What is Braden scale.

It is a very important scale.

To predict pressure ulcer risk.

Any bed bound or wheelchair bound.

Will develop pressure ulcer if

No proper position changes.

Change my position.

Change his position.

They can’t do it by themselves.

Change their positions.
