B.The nurse manager.

C.The Primary care physician.

D. On call doctor.

Correct answer is C.

Rationale :The Primary care physician knows the patient well.A,B,and D are wrong. On call doctor only covers when the PCP is not around and doesn’t know the patient. So call the primary doctor first.
