Vancomycin 500mg IV during Hemodialysis. Zosyn 3.375gm IV Q8hrs.

Norco 10/325mg po Q6hours prn pain.

Multivitamin 1tablet po Daily.

Folic acid 1tablet po daily.

Saline lock right forearm patent.

MRSA Swab and Belongings list done in Emergency room.
