WHAT IS COW PROTOCOL?.

COW means Clinical Opoid Withdrawal. COWS= Clinical Opoid Withdrawal Score.COW score is use for assessment and treatment of drugs Withdrawal.COW Scale is use for management of Opoid use disorder and Buprenorphine is the drug of choice for the treatment of OPoid disorder.

Drugs abusers uses drugs such as Fentanyl,Methamphetamine ,Xanax and other opiates.In every prescription of Opiate there is always warning from pharmacy that says “CAUTION:OPOID RISK OF OVERDOSE AND ADDICTION “.

Induction starts 48hours to 72hours following the last drugs used,follow doctor orders and your facility policy and procedures. Medications such as Buprenorphine(Suboxone )are used to start the initial induction depending on doctor and Psychiatric orders.Other medications are available such as ativan,phenobarbital,Benadryl, Toradol ,and Nausea/Vomiting medications such as Phenergan and Zofran if needed.Follow your facility policy and procedures.

COW Scale is use for the measurement of Opiate Withdrawal. Give the first Medication when the COW Score is greater than 7.

5 to 12 is mild withdrawal.

13 to 24 moderate withdrawal.

25 to 36 moderately severe withdrawal.

GOALS OF DRUG DETOXIFICATION:

Eliminate drug abuse.COW Scale of 5 or less in 24 to 36hours is good.Encourage to stay sober.

Elimination of hunger for drugs or cravings. There should be no respiratory depression from medication. COW Score is used to monitor patient response to buprenorphine.

And this can be done by physicians or nurses.
