1.Failure to check patient’s identification bracelet and ask patient his or her name.
2.Use of misprinted, fade or wrong identification bracelet. If the identification bracelet is misprinted, not accurate call admission department speak with the secretary, he or she will make the correction and you can print out the correct identification bracelet in your unit or floor.Place it on your patient’s wrist .Better safe than sorry .
3.Unable to understand why medication is given or unfamiliar with the medication to be given.
4.Leaving medication at the bedside.
5.Carelessly laying patient’s medication down before administration.
6. Poor calculations of drug dosage. Proper measurement of medication to be given is very important.
7.Failure to check with the ordering doctor when the order is not clear.
7b.Orders that are ordered over the phone from the doctor should be read back and verified.
8.Report impaired healthcare professionals to prevent medication errors. Assess,plan,intervene and evaluate.
9. Maintain high standard of care during medication administration.
