10TH CASE SCENARIO(Nursing).

Age / sex : 92years Female.

Floor: Medical 500048

Admitted 03/05/2012.

CODE STATUS: DNR( Do not resuscitate).

Diagnosis: Cardiac Arrest.Fell at home.Anoxic Encephalopathy.

S/p Asystolic Arrest. Acute Respiratory Failure.

PMH: Afib. ,CAD,GERD,CHF,LPIDS,NEOPLASM of PANCREA.At home she went into cardiac arrest. EMS called ,CPR started at home until EMS team arrived. Placed on ventilator.Do not wean off.

Comfort care.Morphine drip 5ml/hour via PCA PUMP.

02 2L NC.

Medications:

Albuterol .

Solumedrol 125mg iv Q8hrs.

Extubated.

Versed 2mg iv

Keppra 1000mg iv

Lactic acid 4.9

0.9NS bolus intravenously given.

IJ( internal jugular):Triple lumen left subclavian.

PLAN: Call family when patient expires.

Vital sign:

BP 78/45

Hr: 67

Respiration: 10

Temperature: 36.4

02 Saturation 93% with 02 at 2liter NC.
