But the secretary documented left knee the next nurse came in copied and pasted left knee even though the doctor had treated right knee for several months. Six months later the patient called for follow-up up appointment. The clinic still have left knee documented. The right ankle was amputated instead of the left ankle. How do you replace amputated ankle?.Think about it.
